This text examines hyperacute xenograft rejection, a humoral immune reaction which destroys a vascularized organ graft within minutes to a few hours. The author summarizes classical concepts concerning the immunologic basis for hyperacute xenograft rejection, including the role of natural antibodies. Recent insights from pathogenesis are considered, as are potential approaches to prevention and treatment. The risk of hyperacute xenograft rejection has been considered an absolute contraindication to transplantation; the author discusses whether, in light of new developments, that view should be changed.
