Although many theories about Ménière's disease exist and much research is being done, its cause remains undetermined and no fundamental treatment method has been established yet. The articles in this special issue range from Ménière's disease epidemiology and clinical research to inner ear biochemistry, regenerative medicine and genetics. Microanalysis of stress-related gene expression and questionnaire analysis in patients indicate stress involvement for the onset of Ménière's disease. Endolymph metabolic disorder was suggested by single nucleotide palymorphisms in the potassium channel genes in patients. The effects of lithium on AQP2 expression in the rat inner ear indicate that the VP-AQP2 system really works for water homeostasis in the inner ear. A new method for drug application is proposed as a means to prevent inner ear damage or to facilitate recovery from inner ear disease. All these contributions are significant records of the state-of-the-art of medical research, and might point the direction towards future developments. The present special issue is essential reading for otorhinolaryngologists who want to keep informed on recent clinical and basic studies on Ménière's disease and inner ear biology.
